I use free software you can download from Avery.  But I don't use Avery paper forms for the inserts/covers.  The best price for those seems to be Bags Unlimited.

You can get Microsoft Word templates for it.  Not sure if that helps.

I'm pretty sure you can do it with MS Word. There are templates.
