Subject advice for CD traycard templates?

I'm looking for someplace online where I can find a basic template for a CD jewel box traycard. I want to be able to add text on the spines and exterior, print it out, and cut it to fit the jewel case. 

There's a lot of fancy Photoshop/InDesign style traycard templates online (so far that's the way I've been able to do it...download a fancy file, open it in Photoshop, add the text, etc.), but I don't want to mess with all that stuff. I just need basic text on a blank sheet of paper...no color, photo editing, etc. There's got to be a way to have those specs on a basic Word document, right? Or maybe some kind of automated online thing, where you just paste in the text in a window or form or something, and hit print, and see it all print out with "cut here" dotted lines or something.

Anybody have any ideas or suggestions?
