Subject iTunes Question

Anyone ever have this happen?  When I run iTunes on my computer (Windows XP), it "loses" the CDs in the drive-- sometimes while one is playing.  I have to close out and open it again to "see the CDs.  It is a pain in the neck.  Sometimes I'm ripping a CD and half way in the CD disappears and I have to start over again.  This generally happens if I click anywhere on iTunes.
