The way BMI handles its accounting of what is  getting
played is that pretty much every terrestrial radio
station in the country is required to log every piece
of music it plays for one three-day period each year. 
You're assigned a three-day logging period, and so
every day of the year, some stations are logging.  We
just finished our BMI logging period last week.  But
there's definitely no system of tracking every song on
every station.  That may be something that will be
possible once all stations convert to digital
transmission.  Right now, though, it's just a VERY
random sampling of three days per year of programming
from each station.    

Here's a good explanation on the BMI website as to how
fees are determined.  For example, there are bonuses
for hit songs and standards, and college and public
radio are treated differently than commercial radio in
the payment structuring.
