It's been mentioned elsewhere many times, I'm sure, but the grandaddy
of all "bum notes" must be Little Stevie Wonder's ENTIRE "Mary had a
little lamb" harmonica break on "Fingertips (Part 2)" which he played
in the wrong key... It came out so excruciatingly wrong that it was,
of course, perfect.

On a far more obscure note, I had an interesting disagreement with
one of the bands on Messthetics --they'd owned one reel to reel tape
in 1978-79, and whenever they got a new song figured out they'd
record it right on top of whatever was already there. So when they
released their first 45 there were fragments of two other songs in
the "mix"... As with Little Stevie's solo, repeated exposure had left
me quite attached to the tape-mangling outro, but sadly the singer
insisted these had to be excised when I digitized it all 20-odd years
later.
