Sam,

The copy I have came in a plain white card sleeve with "imported from
England" wrapped around the shrinkwrap.  I'd suspect that it was a promo
item, although we'd have to find some Beserkley-affiliated character to
confirm things.  I once asked Tommy Dunbar about it and he'd completely
forgotten that it even existed (as had Marty Rudnick up 'til recently :D).
